FWIW, I insist that all users account names fit my scheme, let's just 
say it's first initial last name "jsmith" This way I can most likely 
know who I'm dealing with as long as I don't have too many users or 
too many domains. As someone else said, make the scheme reveal the 
domain it belongs too. jsmithmd would be from the "mydomain.com" 
domain if you have many users or domains.

Of course it will work to do jim1, jim2, jim3, etc. I wouldn't do it 
myself though as I prefer to have some additional identifying info in 
the user name.

Keep in mind that a user need never reveal their user name on the 
system to anyone, only their email address that they wish others to 
use. A user name could be jsmith and they could receive mail 
addressed to mary@xxxxxxxxxx as long as the address "mary@xxxxxxxxxx" 
is listed in the "other addresses" box for the user "jsmith". They'd 
never need to tell anyone that their user name is jsmith.
