Are you filtering using blackholes? Many of the blackholes list the dynamic
ip addresses ranges of many of the ISPs since no real server should have a
dynamic ip.
The other problem is that large amounts of spam come from China and other
points in the far east. This can cause them to be blacklisted in an RBL.
That could also be the other reason.
What you can do is to cat the log and grep it for the users email address.
This will show you the relevant entries that pertain to that transaction.

Am having problems with a user in China trying to send mail through our
mail server in the UK. Relay mail is restricted, POP before SMTP is
enabled, and the user has a valid account on the Post.Office server.
She can retrieve mail, but the mail server will not let her send mail.
The router in the China office does not have a static IP address. Could
this be causing the issue? Any ideas?
