No, I'm using SA out-of-the-box, via a Tenon PO filter.

Are you using the SA bayes db (i.e. sa-learn)? What is your spam
score set at?
It takes a while and you get the odd FP but I find it works great.

On this topic, does SA ever adjust itself; ie: get smarter? If so,
how
(it needs to)?

My users are getting more spam than ever -- anyone else noticed this?
It seem that the SPAMMERS have gotten smarter lately. With 3 RBL's,
SA and ClamAv, filtering 5,000 spam emails daily, my users are still
getting 150 to 200 spam email daily.
I hate to get back to "roll your own" filters because of the false
positives, but its so bad I'm considering such.
