I remember reading in the documentation about a difference between the way
MS IE and NS Nav ask for docs from the server. Apparently IE only makes the
request to the cache while hitting "reload" in Nav makes the server check
the HD. When the cache automatically updates itself, (which can be up to 3
days depending on how you have it set up) then IE will see the new page.

I'm presuming that domain/directory/ is a different request than
domain/directory/index.html and the latter was not cached. If that
workaround does not work for additional index.html updates, then I'll put
money on him using IE to check page updates. Documentation suggests not
using IE to check updates.

If using navigator to check the updates doesn't solve the problem, then this
info just doesn't apply here.
