I've enjoyed using phpBB <http://www.phpbb.com/>.  Very easy to set up, uses
a MySQL backend and extremely customizable PHP setup.  Very nice and well
supported... and it's all nice and open source and free.

(It uses just about any database you want, actually, but since Tenon makes a
build of the MySQL installer right there with iTools, the ready and working
part is even moreso.)
