Eric, it does have a fully-qualified hostname, reverse DNS. Server
name is mm.lifenet.org and it reverses out as so, I tested it. What
does "Set visible_hostname", is this referring to the reverse or is
this something in a config file somewhere?

fully-qualified hostname. Generally, Squid is telling the truth.
In this situation, should have a reverse DNS record for your IP address
that points to your hostname. Once you add this, Squid will stop
complaining.
