MySQL4. Won't start either (upgraded with the Tenon pkg after update to 7.1)
Seems the previous v3 is still running....
Manual start:

phpMyAdmin path doesn't exist.
The alias in VH config wasn't there... Fixed that

3 alias entries in default VH (icon - Manual - cgi-bin)
no alias at all in other VH's.

Directory Index field was empty for all VH's.

In a normal case, this should have worked, unfortunately, it seems like there is a bad binary on your system. /usr/libexec/httpd/mod_hfs_apple.so is defected. I would recommend to check system update to make sure Mac OS X Server is up to date on your system.

Did you have iTools 6.5 installed? There is a corner case that if iTools 6.5 was installed on the system before. You need to make sure that you had run iTools 6.5 installer to keep your Apache 1.3 in working condition. It is possible that if you moved from iTools 6.5 to 7.0, and then there were Mac OS Xsystem updates. This would break the existing Apache 1.3 on your system. Reinstall iTools 6.5 then iTools 7.1 would fix this problem.



In addition, you will always need to run iTools 6.5 installer and iTools 7.1 installer together when there is new Mac OS X system update.



If you never have iTools 6.5 installed, make sure you have /usr/libexec/httpd/mod_hfs_apple.so exist on your system. Hope this helps.
